How it works
How Forms works
- 1
Build a form in your dashboard
Add short text, long text, email, phone, date, number, and photo upload fields to build a quote request, damage claim, or job intake form.
- 2
Share it or embed it on your website
Send the link by text or email, print a QR code, or copy the website embed code from your dashboard.
- 3
Customers fill it out and submit
Customers answer on their phone or computer and attach photos wherever the form asks for them.
- 4
Actionable responses become tasks, and contact details become contacts
When a response needs follow-up, a task lands in your dashboard with the answers and photos attached. An email or phone number creates or matches a contact. Your team gets a Slack notification when new work needs action, if Slack is connected.
Built for the requests that used to mean a phone call
Quote requests, damage claims, and job details all take the same shape: a few specific questions and sometimes a photo. Forms collects them once, without the back-and-forth.
Quote requests
Ask for the vehicle, service, and timing details you need to quote a job, without a round of phone tag.
Damage claims
Photo upload fields let customers show you the damage the moment they notice it.
Job and intake details
Short text, long text, email, phone, date, and number fields cover most intake needs, in any order you choose.
Website embedding
Copy the embed code from your dashboard to add a published form to your website. A plain iframe option works with site builders that block scripts.
Automatic contact creation
Responses with an email or phone number become a contact in your dashboard automatically, matched to an existing customer when there is one.
Follow-up tasks, no manual entry
When a response needs action, the follow-up task is created with the response attached.
Slack alerts for your team
Connect Slack and get notified the moment a submission needs action.
Trust & safety
You control how Forms reach customers
Share a link, print a QR code, or embed a form on your website. Answering Agent does not automatically text or email forms to customers.
- Copyable script and iframe embed codes are available for every saved form
- No automatic texting or emailing of the form to customers: you decide when and how it goes out
- Forms are shared across your whole team, not set up separately per location
- Slack notifications only fire once Slack is connected, so nothing pings a channel that is not set up
Forms vs. phone tag and paper intake sheets
Quote requests and damage claims usually mean a call, a voicemail, or a sticky note. Forms captures the same information once, in writing, with photos attached.
| Answering Agent | Phone calls / paper intake | |
|---|---|---|
| Getting the details | Customer answers once, on their own time | Multiple calls back and forth to get specifics |
| Photos | Attached directly to the response | Texted separately, if they show up at all |
| Follow-up | Contact and task created automatically | A sticky note or a memory of "I will call them back" |
| Team access | Every location on the team shares the same forms | Depends on who answered the phone |
One platform
The same AI front office on every channel
Forms is one doorway. Phone, website chat, text, and email all use the same business knowledge and land in the same dashboard.
Frequently asked questions
What is Answering Agent Forms?
Forms is an intake tool for quote requests, damage claims, and job details. Build a form in your dashboard, share the link or embed it on your website, and actionable responses become follow-up tasks. Email or phone details create or match contacts, photos included.
Can I embed it on my website?
Yes. Copy the website embed code from the form builder and paste it where the form should appear. There is also a plain iframe option for site builders that block scripts.
Does it text or email the form automatically?
No. Answering Agent does not send the form to customers on your behalf over SMS or email. You control exactly when and how the link goes out.
What fields can I add to a form?
Short text, long text, email, phone, date, number, and photo upload. Dropdowns, checkboxes, and signature fields are not available today.
What happens after someone submits a form?
Answering Agent reviews the response and creates a follow-up task when your team needs to act, with the answers and any photos attached. An email or phone number creates or matches a contact. If Slack is connected, your team gets notified when new work needs action.
Can each location have its own forms?
No. Forms are team-scoped, so every location on your team shares the same set of forms rather than each location building its own.
Is Forms included in my plan?
Forms is a paid add-on channel. Team owners and admins can activate it from Forms in the dashboard, or ask their Answering Agent contact for help.
